Thank you for considering a referral for Neuropsychological Testing for your patient. Our goal is efficient, quality assessment that aids in diagnosis, treatment, and whole-person care.
Dr. Laurin believes that early, accurate assessment to inform treatment must also be timely. Patients should not have to wait 6-12 months for evaluation. To increase efficiency, our model is different. We rely primarily on reaching patients via telehealth. There is a wealth of research to support the validity of telehealth for this purpose (see Other Resources), and patients receive a FREE remote consultation to ensure that they are comfortable with the process and technology before scheduling a testing appointment. An appointment in the patient’s home is also an option.
Dr. Laurin is considered in network with Traditional Medicare (not Medicare Advantage Plans) and Wellmark BCBS Plans. Otherwise, services are considered private pay.
Services are currently available for adult patients (generally aged 50+) located in Iowa.
To refer a patient for neuropsychological assessment of memory and general cognition, simply complete the Referral Form below along with supporting documents (e.g., recent progress notes, in-office cognitive screening, relevant imaging/lab, etc.).
