How do I refer a patient for memory testing?

Do I need a referral for memory testing?

No, you do not need a referral. If you are interested in an appointment for memory testing, please schedule a consultation appointment and/or call/email with any questions. 

In order to have the most thorough assessment, it is helpful to have some basic medical records and information. Ideally, we would like copies of recent clinic progress notes, discharge summaries (from recent hospitalizations), and any brain imaging or lab testing.  You can sign a release to have your doctor(s) fax copies of medical records directly to Dr. Laurin at (515) 226-7981.  It is also important to have a list of your current medications.

What is the process for memory testing?

Schedule a FREE consultation session. As the assessment typically occurs via telehealth, a 10-15 minute appointment is used to discuss the process of assessment, test the technology, and ensure that you are confident and comfortable with the telehealth platform. It is strongly recommended that you have a close friend or family member with you for this consultation session.

Once a consultation appointment is scheduled, you will be sent HIPPA and telehealth consent forms. These forms are required to be signed prior to the appointment.

Following consultation, if you decide to proceed with an assessment, you will schedule a 2-hour appointment. Once scheduled, you will receive Intake and Release of Information forms. Although not required, it is strongly recommended that you have some basic information sent from your primary healthcare provider’s office. This includes recent progress notes, imaging and/or lab results, and a medication list. You can request that this information be sent by talking with your medical provider and/or using the form below.

The assessment appointment lasts up to 2 hours and consists of interview, testing, and discussion. As above, it is strongly recommended that you have a close friend or family member available to participate in a portion of this assessment (at least the first 30 minutes). This assists Dr. Laurin in understanding what others in your life notice about your difficulties or concerns. It is required that you be in a quiet room, free of distractions for the appointment. Please log in at least 10 minutes before your scheduled time.

Please allow 1-2 weeks for scoring, interpretation, and reporting of results. Once complete, you will receive a copy of the report. This will contain a review of history, test results, a summary of findings and diagnosis, and a list of recommendations. A report will also be sent directly to your primary care or referring healthcare provider (unless requested otherwise).

Can I use insurance for testing or therapy?

Dr. Laurin is considered in network with Blue Cross Blue Shield (BCBS) and traditional Medicare (not Medicare Advantage plans). As a courtesy, insurance claims for services will be filed with Medicare and/or BCBS. Patients are responsible for copayments, unmet deductibles, or other uncovered fees.

Outside of Medicare and BCBS, services are considered private pay. If your insurance company provides out-of-network benefits, you may be able to submit for reimbursement. Superbills are provided at your request for this purpose. Patients are responsible for inquiring about benefits and submitting for reimbursement.

What are the costs of testing or therapy?

Memory Testing – Range is typically $800-$1000

Therapy – Initial Appointment/Interview ($250); 50-minute sessions ($150 – $175) 

You will be asked for a credit card at the time of booking, and a deposit will be charged ($150 for testing; $75 for therapy). Unless using insurance (as above), the balance will be charged on the day of the appointment. Deposits will NOT be returned in cases of late cancellation (within 24 hours of appointment).
